2010 Roberta Scofield Memorial Certification Award
This award recognizes nurses who are committed to oncology nursing. Up to 75 nominees will receive a free registration for a 2010 ONCC certification examination or renewal by Option 1 (Oncology Nursing Certification Points Renewal Option). The non-transferrable awards must be used for a 2010 ONCC certification registration.
2010 Roberta Scofield Award Nomination Forms
Award Criteria
- Candidates must demonstrate dedication to oncology nursing and a commitment to obtaining or renewing oncology nursing certification.
- Candidates must be a current and active member of the Oncology Nursing Society (ONS) or the Association of Pediatric Oncology/Hematology Nurses (APHON).
Nomination Instructions
- Each nomination must include:
- A completed Nominee Information Form
- A completed Biographical Sketch Information Form
- A completed Essay Form. The essay, of up to 250 word, must state the nominee’s goals and how oncology nursing certification will help or has helped the nominee realize these goals.
- All forms must be typed. Download the application forms in Microsoft® Word or retype in similar format. Complete each form and return by mail, fax, or email to ONCC. If submitting by email, please include the words “Scofield Award (Your name)” in the subject line.
- Electronic signatures are acceptable.
- Completed nomination packets must be received by August 1, 2009. Incomplete or late entries will not be considered.
- Additional materials or supporting documentation will not be considered.
- All decisions of the ONCC Recognition Subcommittee and Board of Directors are final.
- Members of the ONCC Recognition Subcommittee and Board of Directors are not eligible to be nominees or to nominate individuals.
- Award recipients must agree to be recognized in the ONCC Certification News and/or other publications approved by ONCC. Excerpts of essays may be reprinted in ONCC materials.
- Recipients will be notified by mail in Fall 2009.
