2009 Roberta Scofield Memorial Certification Award

The Oncology Nursing Certification Corporation is calling for nominations for the 2009 Roberta Scofield Memorial Certification Award. This annual award recognizes nurses who are committed to oncology nursing. Up to 75 nominees will receive a free registration for a 2009 oncology nursing certification examination or renewal by ONC-PRO. The non-transferrable awards must be used for a 2009 registration for an ONCC certification test or renewal. A list of recipients will appear in an issue of ONCC Certification News.
Award Criteria

• Candidates must demonstrate dedication to oncology nursing and a commitment to obtaining or renewing oncology nursing certification.
• Candidates must be a current and active member of the Oncology Nursing Society or the Association of Pediatric Oncology Nurses.

Nomination Instructions

• Each nominee must submit the following:
• An essay of 250 words or less stating his or her professional goals and how oncology nursing certification will help or has helped the nominee realize these goals.
• A completed Nominee Information Form
• A completed Biographical Sketch Information Form
• A signed Release Form.
• All materials must be typed. Download the application forms in Windows Microsoft^® Word format (see below). Complete each form in Microsoft^® Word, print and return by mail or fax, or email the document to ONCC. If submitting by email, please include the words “Scofield Award” in the subject line
• All materials must be submitted together.
• All forms must be signed (electronic signatures are acceptable)
• Completed nomination packets must be received by mail, fax, or email no later than August 1, 2008.
• Incomplete or late entries will not be considered.
• Additional materials or supporting documentation will not be considered.
• All decisions of the ONCC Recognition Subcommittee and Board of Directors are final.
• Members of the ONCC Recognition Subcommittee and Board of Directors are not eligible to be nominees or to nominate individuals.
• Award recipients must agree to be recognized in the ONCC Certification News and/or other publications approved by ONCC.
• Recipients will be notified by mail in Fall 2008.
